We currently offer two options to submit your church expenses for approval and reimbursement:

 

  1. Print the Expense Reimbursement form on your computer.  Fill in the form manually and hand in to the Treasurer with your expense receipts. Or, mail completed form, with receipts, to the Treasurer at: Rest Haven SDA Church, PO Box 2126, Sidney, BC, V8L 3S6.
  2. Fillable form coming soon. Fill out this fillable Expense Reimbursement form and attach to an email with receipts to Treasurer:
